L322 Transfailsafe and maybe clutch slip after gearbox oil change

03 Vogue with 120k on original gearbox. Had oil and filters changed on the gearbox two years ago after getting the cattle grid noise occasionally from the rear of the car. Oil change solved it but used to get gearbox squeal for a few mins when cold. Local shop checked oil level and said it was fine but oil was filthy already. Changed it again last month and the squeal has disappeared but now occasionally I get a transfailsafe warning, usually soon after reversing the car.

More worringly, sometimes the car feels really underpowered or that the clutch is slipping somehow. Its not limp mode as there's more power available than that, but is seems to struggle to pick up speed from a standing start. Once moving it seems ok and pulling over and restarting the car fixes it straight away.

Any ideas what this would be? Is it a sign that the gearbox/clutch is on its way out?

One of the most common issues in the ZF 5HP box is the o ring between two clutch/drive packs (B and C I think), this reduces available hydraulic pressure to the A clutch and can cause it to slip, this can some times manifest with a fault code along the lines of speed transducer fault or input speed error or something that I cannot remember...but Bemble/RRPhil is the man in the know who can give a much more definitive answer.

Have you had the codes read from the transmission ECU to see what the logged trans failsafe code is?
