L322 4.4 M62 running fault

I have owned my 2003 l322 4.4 M62 Petrol for 5 years and done 50,000 miles in it. It is now it's 160,000 miles. In this time it just been entirely troubled free! Everything still works! But now I have a fault and it's a pig!!!

Over the last month the car started coughing/hiccupping whilst driving or whilst maneuvering a trailer at low speed. The cough is now developed into an inability to to idle at all on first startup and when you press the accelerator it the engine dies. It is now undriveable.

The engine management light is not on.
There are no fault codes
I thought it might be an air leak but the intake hose is good.
I fitted a new air mass metre but no improvement.
I am now stuck, Any ideas?
 
Update on this:

Having watched a couple of videos and the excellent advice here, I began to suspect a fueling issue. I purchased a fuel pressure gauge and sure enough, the pressure was low. I bridged the relay for the fuel pump (BMW pin pattern - completely different to a normal relay) and the pump also sounded very rough indeed. So new pump ordered and will also replace the filter/regulator. This might not be the only issue but replacing these bits should move me forward!
