Ipod cassette tape in td5

Would say that either the tape deck or the cassette is faulty, it should just turn the spindle and never see any tension,this is used to swap sides / eject the tape. I think the head unit is detecting tension and therefore causing your problem
 
Most Tape decks work in a way that they should both detect that the Supply and takeup
spindles are turning. they have sensors underneath!
when a normal cassette is inserted the tape pulls the the takeup spindle causing it to turn
In your case because the holes are free this would cause the unit to autorev or eject because it's thinking that the tape has reached the end or been chewed up.

Another issue may be the volume from you ipod / mp3 player. Make sure it is turned up most of the way, as the deck in my 01 ES will start fast forwarding if no sound is coming from the tape connection (think that can be solved by pressing the MAN button though.)
