Fan belt keeps flipping over

Had the RAC out yesterday as I'd heard the fan belt slipping a bit, and then the steering went really heavy and battery light came on. RAC noticed it had flipped around, so the teeth were on the outside. He put it back and tightened it up. Started it this morning and same problem, checked under bonnet and it's flipped over again. Tried to turn it round but it seems pretty tight. Any ideas?
 
I had a ride on lawnmower that kept doing this to its drive belts too.

Is it a flat or v belt? As above, new belt is indicated, but have a look at the pulleys - is anything misshapen or running out of line? I've had situations with pulleys breaking up in the centre so they're not running true, and bearings wearing so they're skewed in relation to the other pulleys.
 
must be a Vee belt, no way a serpentine could flip surely?
Otherwise as above, new belt and check everythings in line/not wobbling etc.
 
you get the roll over of the belt usually when some of the cords fracture inside the rubber part , so that it wont hold its inside smaller than the outer circumference
 
I saw this as a RAC Patrol on a diesel transit once. the ribs were as someone said warped also acting like a thread and unwinding the belt. Watching TV I saw how they make these belts. so the belt could be wrong from the start and then when worn down, it just gives up. Lucky it's not a cambelt. Although I used to change both belts when I did a change. Easier to see the condition and judge the aging accordingly.
