Crank gear can't be in the wrong place and cam gear can't be wrong. That leaves the injection pump which has a gasket (although I can't see that makes a difference) and also has the pump rear support bracket. So I can't really see how that would be wrong.
Could the actual timing case play a roll in all this somewhere, it's a 2nd hand one from Turners, could it be out of whack somewhere?

Try and find the instructions for the 300 belt mods, as Iirc there was some things you had to do when fitting the inj pump.
 
Using my Heath Robinson straight edge the cam gear and crank gear are good.
Using it across the cam gear and injection pump gear it suggests the outer edge (nearest the outer edge of the casing) of the pump gear is further back. There is a 1.6mm gap between the gear and straight edge, measured with feeler gauges.
Would that suggest a warped timing case?

Could be the inj pump or the timing chest that is not square, ie something stopping either one seating correctly.
Remember cam and crank are square as they are in the block, inj pump and tensioner rely on the timing chest.

I'm going to remove the injection pump gear and pump and reseat them tomorrow and see if I can see anything a miss. Quite what could be wrong I don't know.

Straight edge across front face of timing chest and measure from straight edge to the face of the gears?
Using same straight edge see if the timing chest itself is bent.
 
I have tried three different timing cases and they all fit and measure up the same, injection pump gear is not quite in alignment, I can't see they all can be wrong.
Injection pump is newly rebuilt but can't really see how it could be wrong.
Anyway its all back together now with a different tensioner and a 3rd timing case.
I spent a good half hour turning it over by hand watching the belt and it stayed central to the tensioner pulley. I know that doesn't really mean anything but what else can do.
Fingers crossed and will keep an eye out for anything coming out the wading plug hole.
On the upside it all came apart easy!

Interesting, if anything our pump gear needs to go forward.
I notice the 300tdi has bushes in the bracket whole the 200tdi has a fixed bracket.
I bolted the pump to the timing case and left the bracket loose, tensioned the belt up and then did the bracket up.
There isn't really much adjustment in it to be honest.
If its a problem again I will see if I can find a local engineering shop, if they exist anymore.
