    Not sure how much water it's using over what period of time - but it could be the water resevoir. I seem to recall these crack (very slightly) in fact I think mine has. I only have to top up with perhaps a pint or so every month or two (depending on miles driven).

    I suspect 2 things here - sounds like your fuel pressure is causing a problem that's resulting the fault code. New filter would do no harm, but it could be pump related also so check for fuel pressure as greenie suggests. However, as Hollie mentioned I also think there's some auto-box issues...
