I do believe the sticky out thing is just a drain if it fails. I repaired two with a repair kit and both have been successful, no idea if it's your problem, ours were leaking but drove ok. Fuel pumps do whine in general in my experience.

Yes you certainly need an air filter. Does it have a radiator fan? Looks like the wiring needs a bit of attention. Cambelt change and a good service of all fluids, keep you busy for a while. Has it been sat around for a while?
