I have heard about this problem from many people but haven't found a definitive solution.

Between 2000 and 2250 revs in 4th (Disco 2 Auto 2002) while going up hill, i.e. under load the hole car starts to shudder quite violently. It seams to do it less when warm and it only does it in 4th. Sometimes it seams to be coming from the front of the car and sometimes the back at other times its the whole car.
The UJ's appear to be fine There is a small amount of play in the front output shaft bearing, but no worse than any other Disco. The AMF is ok and there is no oil in the injector harness. Wheel bearings are all good and wheels all balanced.

Any ideas?

Could be a bent prop or damaged trans brake. Dont think a uj that feels ok is actually 100% they can seize a bit if not greased. Sliding joint should move nicely on the prop this can cause vibes. My front prop did this only in 3rd uphill. New ujs didnt fix it, sliding joint fine alignment fine. New prop sorted it, who knows why!
Cv joint could cause it but would have clicking on steering at liw speed. Prop donut another culprit or transfer box can sometimes cause vibes if something knocked out of true. Try taking front prop off and running in difflock to test, ignore the backlash and see if it cures the vibes.
 
The shudder fix stuff looks interesting I might give that a go, I haven't changed the transmission fluid as the guy who owned it before me said he had done it, however I am not sure now that he did so its something I will do.

Transmission aside I think I may have solved the problem. When I bought the car I asked the guy about the injector loom and he said it was fine. He even took it off and showed me! Just as a double check I I took it off again today and had a look. Sure enough its full of oil. I suspect the last owner cleaned it before I came around so that it wouldn't show up on the test drive. So I will replace the loom and see what that does.

Another possible is the waste gate on the turbo. These can seize causing over pressure faults which causes the ECU to cut fueling.

I will also have another look at the prop-shaft etc but alas have no diff lock so cant take the front prop off.
