Il try adjusting the valves first. The hot spots are definitely in all the way took a 2.5lb hammer and a block of wood to get them in. Then I peened them in with a centre pop
 
Found this on number 4 cylinder I hope bits of this aren't the cause of the knocking

Had another go at the tappets engine is a lot quieter but still not right. Think I need another glow plug resistor as it was smoking and the glow plug light was really bright. Have a listen folks
 
Think I'm just really crap at doing valve clearances. Should add I'm using an imperial feeler gauge with the Blade marked 10 is this correct?

Thought it was yeah I know the one you mean I watched that. I'm definitely thinking it's a valve clearances as it's better now but I just can't seem to get them quite right. I think I'l have to go a smidge tighter to account for the wear on the rocker pads and my feeler gauge being a bit pitted. Hot spots wouldn't account for the diesel smelling smoke I don't think

if you cant get tappet setting right you may have worn roller cam followers, each tappet adjuster should have similar threads above the nut if one or more vary much theres an issue with push rod or followers
 
Il have the rocker cover off again tommorow and have a look I did check all pushrods for straightness and they were fine all seated correctly too. All I can think of is to tighten them up a wee bit. The noise is more pronounced on the left hand side

Had another go today reduced the clearances to 8 thou much much better but still not quite there il try 7 thou. I think with mine having 190k of wear to the valve train 10 thou might as well be 10 miles! I did notice on the plate on the rocker cover telling you the valve clearances has had the numbers scratched off. Think I know why now lol
 
It's definitely tappet related it didn't do it before so haven't changed anything valve gear related but the rocker pads did have grooves in them. Ive set it now 4 times slightly tighter each time and it gets better all the adjuster screws seem at similar heights. You can hear it with a long extension to the rocker cover to ear. Should there be little caps between the valve stems and the rocker pads? If so they were missing on mine

Update. Think iv found the problem being the last rocker shaft pinch Bolt nearest the bulkhead was stripped somebody had jammed bits of copper wire into the hole lol so iv ordered a 5/16 unf helicoil the Bolt is fine. I take it this would account for the rattle? Never know what you'll find taking an old engine apart!
