If someone accepted a discount for having a dash cam and for some reason it was not working i.e. forgot to start it up, what would be the position of the insurer? would the insurance be totally void in any accident?

Only if the Dashcam is issued by the insurance company as part of the package. That way the driver is obliged to record when in use. I suspect recorded material is the property of the insurance company. Its like the same with having a blackbox recording your traffic movements and the same if you own a hgv with tacograph.
