Both, but only the belt needs changing every 10 years or 150K miles. Preferably by someone who knows what they are doing.
I suspect my bent inlet valves were caused by the PO not knowing how to do the job, or not using the £25 locking tools.
The chain links the exhaust cam to the inlet cam, but it's not a service item, unless it goes loose and rattles on starting.
