I am still a bit confused why you are holding the key between ignition and crank. If it's a 200tdi then it has a heater-plug relay, only the (older engines) 19j needs the 'hold'
The 200A requirement is for the heater plugs

Yep you will need a new cover.
Remove pump off the rear of the transfer box & there may be a gear you need to remove as well.
You may need a diffent out put gear depending on the above.
