This weekend has me updating the shifter on the Defender 90.

Having taken the top section off, I’m greeted with this sight.

A) square headed bolt in place of the original grub screw (easy switch once I can get the bloody thing out)

B) a combination of what looks like grease, oil and/or mud swimming in the unit

Is this normal or should I clear this out before fitting the new parts?

It's normal.
Here's mine.

I traced most of my ingress to the O ring seals on the H/L lever. The red fluid is Dexron III from the gearbox, it seems to travel along the selector rod and settle in the cavities.
Hopefully iv'e cured that too with new O rings on the selector rod.

Is that an LT77 gearbox ?
What engine/year is your car ?

At β€˜91 it should be an lt77. The exact variant you will need the serial number. Easy way to tell is lt77 reverse is left of first, r380 reverse is right of 5th and Transit gearboxes have 6 gears which I know nothing about.

In terms of your original question as above perfectly normal. Gove it a good clean and nice large blob of what ever heavy grease you have on the shelf before reassembly.
