Driving home from Wales this afternoon in my Disco 2 Td5 i noticed my back windscreen started to look dirty, used the washers and it looked greasy... I could also smell diesel with the windows open.

Got home three hrs later and the whole back end is covered in diesel, also the chassis and tow bar, it was dripping off the tank and left a big puddle in about 5 minutes :-(

It seemed to stop after half an hour. Basically there's fuel everywhere over the tank, and exhaust. I read that it's 99% likely to be a split green fuel hose ? Any thoughts ?

If so is it easy (ish) to change.

I make take it to my local friendly garage as I think it'll need a lift to get it off the ground high enough. Also sod's law it's 3/4 full of fuel too.

If you get a chance to look ... check the rear chassis and tank supports while your under it ...
 
If you pull up the carpet in the rear loadspace you should find a circular inspection panel which is directly above the fuel pump where you might be able to gain access to some of the pipes.
If you have ACE, you should also check for any leaks on the pipes to and around the rear active anti-roll bar; that can give similar symptoms to a leaking fuel system.
 
Had the same problem a few months ago - faffed around with the bolts holding the tank in place but gave up with that.
Instead I located the green high pressure fuel pipe which is located behind the fuel tank (towards the engine) and followed it towards the fuel tank un-clipping the pipe as I went (2-3 clips).
Quite straight forward, no ramps or jacks required - but you've got to do it by feel.
