Anyone seen this before!?

I'd hope as the driver I would have picked up the play in the steering around straight ahead before a ball joint separated ;). Or has it actually broken ?

I had this happen to me after a 70mph trip down the M5. No difference to steering prior to failure and had been MOT'd two months before. Actually just fell off when I parked up, very scary. Wrote to Landrover suggesting the should fit grease nipples to these joints. Predictably they didn't want to know.

...has the steel tapered pin detached from the ball? I can't tell if the complete ball joint has come out. Cheap ball joint?

Standard height or lifted?
The problem should have become apparent long before the falling out.
I have had the same thing happen on an old workshop Hilux that was pretty flogged, certainly makes for an interesting driving experience at 100kph on a bad dirt road, sh1ts were certainly trumps that day.

Never seen a D2 pitman arm up close, are they all banana shaped? Does the D2 use the same steering box as a P38 or the 4 bolt Adwest unit? Seems strange unless the steering bar is deliberately longer ala P38?
